Summary of Position The Health Department's Public Health Bureau is seeking to fill vacant Supervising Public Health Nurse positions in the Community Based Nursing, Communicable Disease Prevention and Control, and Children's Medical Services divisions.
 
The Supervising Public Health Nurse (SPHN) coordinates and supervises the operation of health-related services and programs involving comprehensive case management, education, prevention, and collaboration with and coordination of community resources. These services are delivered by a team of professional, para-professional and support staff that collaborate to ensure comprehensive health services are provided throughout Monterey County. The supervisor uses principles of reflective practice and staff development to provide primary support and appropriate oversight to the nurse home visitors to build and model a strengths-based client-centered culture. These programs include:
 
Nurse-Family Partnership (NFP)/Maternal Child Adolescent Health (MCAH) SPHN: NFP provides specially trained nurses to partner with low-income first-time mothers, beginning at approximately the 28th week of pregnancy, to promote and teach positive health and development behaviors and improve economic self-sufficiency. The program continues to work with the mother and child until the child's second birthday. MCAH works to improve the health and well-being of women, infants, children and adolescents through the provision of community leadership through our stated Scope of Work and nurse case management by assuring access to health promotion and health care services. This program includes the provision of Perinatal Services Coordination and SIDS Coordination.
 
Whole Person Care (WPC)/Targeted Case Management (TCM) SPHN: WPC is tasked with providing nurse case management to our most vulnerable Medi-Cal homeless individuals who are high users of hospital, mental health unit and emergency department facilities. The program partners with community organizations and primary care providers to help these individuals find a medical home, housing, social and mental health services. TCM provides specialized nurse case management services to Medi-Cal eligible individuals in a defined target population to gain access to needed medical, social, educational and other services.
 
Tuberculosis (TB) Control Unit SPHN: The TB Unit has the primary responsibility for medical case management of individuals diagnosed with active TB disease.  The Unit works to prevent additional TB transmission, ensure an adequate course of treatment is followed, and identify and address other urgent medical and psychosocial needs through appropriate utilization of resources.  Other responsibilities include conducting contact investigations, educational outreach, and informing medical providers of current screening and treatment recommendations.  

Communicable Disease Unit (CDU) SPHN: The CD Unit provides communicable disease prevention and control services for all Monterey County residents.  Program activities include conducting detailed interviews, collecting biological specimens, delivering health education, linking clients to treatment and/or preventative services, and providing guidance to medical providers, schools and congregate living facilities on screening, treatment and infection control practices.
 
California Children's Services (CCS)/Child Health Disability Prevention (CHDP)/Child Lead Poisoning Prevention Program (CLPPP) SPHN: CCS determines financial and medical eligibility for children under 21 years of age for CCS-eligible diagnostic and treatment services, medical case management and physical and occupational therapy services. CHDP provides care coordination to assure periodic health assessments and services to low income children and youth in California, and administers the Health Care Program for Children in Foster Care (HCPCFC) program that is co-located in the Department of Social Services and Probation to provide public health nurse expertise in meeting the medical, dental, mental and development needs of children and youth in foster care, including psychotropic medication monitoring and oversight (PMM&O). CLPPP seeks to prevent, screen, diagnose and treat childhood lead poisoning through nurse case management, and to eliminate sources of poisoning through prevention education and environmental interventions.

Will have a proven track record demonstrating the following knowledge, skills, and abilities :  
 
  • Knowledge in principles methods, and procedures of public health nursing.
  • Knowledge of principles and practices used in basic budget and grant administration.
  • Ability to understand interpret and apply rules, laws, policies, and regulations relative to specific public health programs.
  • Knowledge of community aspects needs and resources relative to specific public health programs.
  • Knowledge of the principles, techniques, methods and terminology used in specific medical specialties which includes case management, maternal/child and adolescent health, nutrition, foster care, immunization and communicable disease control.
  • Ability to evaluate and draw logical conclusions from data; evaluate problems, policies, procedures and formulate an effective course action.
  • Ability to plan, coordinate and implement overall work plans to achieve a designated objective; coordinate multi-faceted projects and studies, including coordination and planning for resources, manpower and timing.

Conditions of Employment :
 
  • Pursuant to Title 17 of the California Administrative Code, possession of a license in active status, to practice as a registered nurse in California and possession of a Public Health Nursing Certificate.

The knowledge, skills and abilities listed above may be acquired through various types of experience, education or training, typically:
 
Education: Completion of coursework leading to a bachelor's degree in Public Health Nursing which included courses in the principles of supervision and administration and Supervising Public Health Nurse
 AND
 
Experience: Two years of progressively responsible professional public health program experience which include lead-worker level responsibilities, and which provide a broad knowledge of public health programs.
